Main Duties & Accountabilities

As a Resettlement Case Manager your main accountabilities will include:

·         To provide Resettlement services to applicable offenders under the line management of the Resettlement Coordinator and in line with the  service specification.

·         Adopt BIONIC (Believe It Or Not I Care) ethos

·         Deliver 1-1 and group support / interventions as required.

·         To provide advice, information and support to multi agency workers and offenders about the nature of the Resettlement Services provided.

·         In liaison with the Resettlement Co-ordinator, participate in the development and delivery against team objectives.

·         Monitor all reception and release information to ensure offenders are case managed timely to their sentence length, have access/are referred to a range of resettlement interventions and are fast tracked, where appropriate, to release support planning for short sentenced offenders.

·         Undertake assessments of prisoners and work with prison and other colleagues to address immediate needs.

·         Undertake assessments and produce Resettlement Plans and reviews engaging with each offender and taking account of their risks, needs and capacity, opportunity and motivation to stop offending.

·         Inform, consult and involve each offender in determining rehabilitation plans and activities.

·         Work with colleagues and partners in prison and the community and engaging peer and volunteer mentors where provided to ensure that offenders comply with agreed activities and enable access to resources that support rehabilitation.

·         Actively promote equalities and diversity taking account of each offenders’ different needs.

·         Manage risk using prison procedures in appropriate cases.

·         Represent the Service in relevant prison meetings.

·         Maintain records in accordance with national, prison and CRC policies and procedures.

·         Use relevant information and technology appropriately with proper attention to data security.

·         Engage in supervision and appraisal meetings with the line manager and take opportunities to undertake further personal development activities.

·         Contribute to plans and developments of the prison, CRC and relevant partnerships

·         Deliver a group work programme (Making a Change – MAC) to prisoners in the custodial setting.

·         Engaging and motivating prisoners to take part in sessions and delivering practical skills around several pathways to support a reduction in reoffending.

A Resettlement Case Worker liaises closely with key stakeholders such as prisoners, Probation staff, Prison staff, Housing Officers and workers for other statutory and voluntary agencies both within and external to the prison who offer services relevant to the resettlement of prisoners.
