Purpose of the job

To   provide housing related support to vulnerable individuals together with providing housing and property management with the aim of achieving a successful move-on and other measurable outcomes.

Duties and responsibilities

1.    Engage with actual and potential referral agencies including local authority central access point/gateway services, statutory and voluntary sector agencies in order to generate referrals for the service as required.

2.    Carry out an initial assessment of the potential or actual service user, including any risks, of his/her needs and requirements in respect of the service, to determine eligibility for the service.

3.    Design and deliver an effective support plan with the service user referring to specialist agencies as applicable. This should be based on an initial assessment of the service user’s needs and abilities. The progressions/outcomes delivered should be recorded, monitored, reviewed and updated regularly and in line with contract requirements.

4.    Maintain contact on a regular basis with service users in accordance with the requirements of the service and no less than the requirements of the minimum contact procedure. This includes maintaining a professional relationship and being a good role model to service users. All contacts should be appropriately recorded.

5.    Regularly update service users risk assessments, taking any necessary action as required.

6.    Ensure that, where applicable, housing and property management tasks are effectively undertaken in line with procedure and targets. 

7.    To ensure that routine property maintenance and damage is reported promptly to maintenance staff and to co-operate with them in meeting repair response times.

8.    To ensure that properties are always adequately equipped, furnished, and cleaned, by organising, purchasing or working with others on practical tasks. Where required, the cleaning and equipping of properties is to be undertaken by Project Workers.

9.    To ensure the correct tenure and support agreement is issued and service users comply with these agreements, taking appropriate action in respect of non compliance including non engagement with support.

10. Ensure that void time is minimised by taking a pro-active approach to ensure properties are re-let as soon as possible in line with targets. 

11. Maximise rent collection, ensuring  successful housing and other welfare benefit claims are submitted and maintained, taking proactive action to minimise rent arrears in line with targets, monitoring the account and recording all actions and where necessary taking appropriate action when arrears arise in line with policy and procedure.

12. Actively engage with service users in decisions that affect them, informing them of their procedural and representational rights, ensuring they have input in shaping the nature of services delivered to them through involvement activities ranging from providing information, requesting feedback, consulting to facilitating involvement by the service users.

13. Empower and motivate service users to Identify and maintain focus on achievement of the desired outcomes from the service being delivered.  Ensuring the service user is assisted to prepare for and participate in suitable opportunities including educations, training, volunteering and employment.

14. Support service users to engage in meaningful constructive activities to make effective use of their time and develop their confidence and skills.

15. Deliver practical life skill sessions, programmes with service users as required.

16. Develop a move-on plan with service users at the earliest opportunity, identifying realistic options for independent or more appropriate housing.

17. Work collaboratively in a multi-agency setting as required. This includes maintaining professional relationships and appropriate confidentiality of information.

18. Meet individual and contribute towards meeting team performance targets in respect of the service.

General

1.    Engage fully in regular supervision with your line manager.

2.    Ensure individual expertise and subject area knowledge is up to date through Continuous Personal Development, including sharing good practice, engaging with training and acquiring / maintaining any individual qualifications required of the role.

3.    As necessary, and in addition to the above, undertake other activities commensurate with the nature of the post including taking on a concurrent responsibility that supports delivery across one or more teams.
